Patient Screening Tools Now Online

Princeton Nassau Pediatrics is proud to announce that we are implementing a new interactive online screening tool designed to provide your children with the best care possible.  The screening tools will help assist us in the early diagnosis of developmental, social, emotional, and behavioral concerns as well as a variety of health risks that can be difficult to see during a typical visit. All responses are completely confidential and cannot be viewed by anyone except your doctor.

These screening tools will save you time during the office visit and allow our physicians to spend more time directly with our patients.

Initially, patients who have their 9, 18, 24 and 30 months well visits will receive the SWYC (Survey of Well-Being in Young Children) screening tool.  Pre-teens and teens, 11 and up, will receive the PHQ-9 (depression screening) at their well visits.

Both of these screens are recommended by the American Academy of Pediatrics and Bright Future guidelines and are the same screening tests we have been using in the office for years. We are now just able to offer it in advance of the visit, so you can fill it out thoughtfully and not feel rushed.

For parents of teenagers:  The PHQ-9 (depression screen) is meant to be filled out independently by your teen. Sometimes younger pre-teens need help answering the questions, but usually, teens can and should fill these out independently. This allows teenagers to actively participate in their own healthcare and empowers them to provide honest and open answers. Allowing teenagers to answer questionnaires independently is crucial to our shared goal of providing the best care possible for your child.

These questionnaires will be sent directly to your patient portal and you will receive notification that they are there. If you have not signed up for our patient portal, please do so at your earliest convenience.
